RESUMO

Objective: To clarify the effectiveness and safety of psycho-behavioral intervention combined with a non-benzodiazepine to improve perioperative sleep quality in patients undergoing knee arthroplasty, and also to explore the optimal dosage of non-benzodiazepine (oral zolpidem tartrate) to form a standardized sleep quality management process to promote accelerated recovery of patients. Methods: 240 patients undergoing initial unilateral total knee arthroplasty between January and December 2016 were prospectively included and randomly divided into blank control group (group A), psycho-behavioral intervention group (group B), zolpidem tartrate 10 mg group (group C), and psycho-behavioral intervention + zolpidem tartrate 5 mg group (group D). Sleep, pain, psychological, and knee function scores were compared. Results: There were significant differences between the four groups with respect to Pittsburgh sleep quality index scores, sleep efficiency, Epworth Sleepiness Scale scores, inflammatory indices, pain, and enhanced recovery after surgery indices during hospitalization (P < 0.05). Conclusion: Psychological behavioral intervention combined with non-benzodiazepine can improve the perioperative quality of sleep. Compared with drug intervention alone, it reduces the dosage of non-benzodiazepine, alleviates anxiety, improves patient satisfaction, and enhances the quality of life.

@#Objective 聽 聽 To explore the application value of failure mode and effect analysis (FMEA) in the risk management of unplanned extubation after esophageal cancer surgery. Methods 聽 聽 A total of 1 140 patients who underwent esophageal cancer surgery in our department from January 2015 to May 2017 were selected as a control group, including 948 males and 192 females with an average age of 64.45卤4.53 years. FMEA was used to analyze the risk management process of unplanned extubation. The potential risk factors in each process were found by calculating the risk priority number (RPN) value, and the improvement plan was formulated for the key process with RPN>125 points. Then 1 117 patients who underwent esophageal cancer surgery from June 2017 to December 2019 were selected as a trial group, including 972 males and 145 females with an average age of 64.60卤5.22 years, and the FMEA risk management mode was applied. Results 聽 聽 The corrective measures were taken to optimize the high-risk process, and the RPN values of 9 high-risk processes were reduced to below 125 points after using FMEA risk management mode. The rate of unplanned extubation in the trial group was lower than that in the control group (P<0.05). Conclusion 聽 聽 The application of FMEA in the risk management of unplanned extubation after esophageal cancer surgery can reduce the rate of unplanned extubation, improve the quality of nursing, and ensure the safety of patients.

ObjectiveTo screen out differentially expressed genes (DEGs) and related candidate therapeutic drugs for acute pancreatitis (AP) using the bioinformatics method. MethodsHigh-throughput microarray datasets (GSE109227 and GSE65146) associated with AP in mice were downloaded from gene expression omnibus, and GEO2R was used to screen out DEGs. Database for Annotation, Visualization and Integrated Discovery was used to perform gene ontology and pathway enrichment analysis. Protein-protein interaction (PPI) was established in String database and visualized by Cytoscape, and then subnetwork modules and hub genes were screened out. The microRNAs associated with the hub genes were predicted and candidate drugs were screened out using Comparative Toxicogenomics Database (CTD). ResultsA total of 130 upregulated and 16 downregulated DEGs were screened out in the high-throughput microarray datasets GSE109227 and GSE65146. DEGs were mainly involved in the biological processes such as inflammatory response, neutrophil chemotaxis, tumor necrosis factor-mediated cellular response, and positive regulation of gene expression, and they were also involved in the signaling pathways of extracellular matrix-receptor interaction, regulation of actin cytoskeleton, leukocyte transendothelial migration, and focal adhesion. A total of 12 hub genes and 6 subnetwork modules were screened out in the PPI network. The microRNAs including miR-199a-5p and miR-1-3p might regulate the post-transcriptional regulation of key genes. Genistein, resveratrol, and quercetin which were screened out from CTD database could reduce the expression of key genes. ConclusionRelated genes screened out by the bioinformatics method may play an important role in the development of AP and can be used as the basis for drug screening.

Purpose@#Fibroblast growth factor receptor 4 (FGFR4) plays a critical role in cancer progression involving in tumor proliferation, invasion, and metastasis. This study clarified the role of FGFR4-Arg388 variant in gastric cancer (GC), and more importantly highlighted the possibility of this single nucleotide polymorphism (SNP) as potential therapeutic targets. @*Materials and Methods@#FGFR4 polymorphism was characterized in advanced GC patients to perform statistical analysis. FGFR4-dependent signal pathways involving cell proliferation, invasion, migration, and resistance to oxaliplatin (OXA) in accordance with the SNP were also assessed in transfected GC cell lines. @*Results@#Among 102 GC patients, the FGFR4-Arg388 patients showed significantly higher tumor stage (p=0.047) and worse overall survival (p=0.033) than the Gly388 patients. Immunohistochemical results showed that FGFR4-Arg388 patients were more likely to have higher vimentin (p=0.025) and p-STAT3 (p=0.009) expression compared with FGFR4-Gly388 patients. In transfected GC cells, the overexpression of FGFR4-Arg388 variant increased proliferation and invasion of GC cells, increasing resistance of GC cells to OXA compared with cells overexpressing the Gly388 allele. @*Conclusion@#The exploration mechanism may be through FGFR4-Arg388/STAT3/epithelial to mesenchymal transition axis regulating pivotal oncogenic properties of GC cells. The FGFR4-Arg388 variant may be a biomarker and a candidate target for adjuvant treatment of GC.

RESUMO

Neonatal hypoxic-ischemic encephalopathy (HIE) is a major cause of neonatal mortality and neurologic sequelae. Current evidence showed that the brain-derived neurotrophic factor (BDNF) could be considered as a specific biomarker of brain injury, which could help to assess the severity and prognosis of neonatal HIE. Also, BDNF may be widely applied in clinical practice to monitor the treatment of neonatal HIE due to its important role in neuronal survival and growth. This paper summarizes the possible role of BDNF in the management of HIE from these aspects.

To test the therapeutic effect of recombinant fusion polypeptide hEGF-AWRK6 (EK) on burn infection of model mice. EK6 was expressed and purified with Escherichia coli expression system, and the Ⅱ degree burns and Pseudomonas aeruginosa infection model mouse were established. Experiment group was treated with EK (30 mg/L), and the control group was treated with PBS, gentamicin (30 mg/L), burn ointment (10 mg/L). The wound healing rate and colony count were calculated. Wound and surrounding skin were taken for HE staining and collagen western-blot analysis, and the wound pathological changes were observed after 10 days of drug delivery. The results showed that fusion peptide EK was successfully expressed and purified with significant antibacterial activities against Pseudomonas aeruginosa. Compared to the control group, the colony count (CFU) of the wound surface in EK mouse had a remarkable decrease (P<0.01) and healing rate had a significant increase in group EK6 (P<0.01). Pathological analysis result showed that compared to the control group, wound dermal cells in group EK arranged regularly, had more hair growth and a faster epithelization. These results indicated that the fusion peptide EK would be a good candidate for the drug development for the treatment of burning wounds.

The organization of the brain functional network is associated with mental fatigue, but little is known about the brain network topology that is modulated by the mental fatigue. In this study, we used the graph theory approach to investigate reconfiguration changes in functional networks of different electroen-cephalography (EEG) bands from 16 subjects performing a simulated driving task. Behavior and brain functional networks were compared between the normal and driving mental fatigue states. The scores of subjective self-reports indicated that 90 min of simulated driving-induced mental fatigue. We observed that coherence was significantly increased in the frontal, central, and temporal brain regions. Furthermore, in the brain network topology metric, significant increases were observed in the clustering coefficient (Cp) for beta, alpha, and delta bands and the character path length (Lp) for all EEG bands. The normalized measures γ showed significant increases in beta, alpha, and delta bands, and λ showed similar patterns in beta and theta bands. These results indicate that functional network topology can shift the network topology structure toward a more economic but less efficient configuration, which suggests low wiring costs in functional networks and disruption of the effective interactions between and across cortical regions during mental fatigue states. Graph theory analysis might be a useful tool for further understanding the neural mechanisms of driving mental fatigue.

Glutamine is not only a special nutritional substance but a conditional essential amino acid,which is the most abundant amino acids in plasma,representing about 60% of free amino acids in the body,and it's normal plasma concentration is from 0.6 to 0.9 mmol/L.In recent years,with the increasing deepening study of the field of clinical nutrition,the concept of glutamine study has been gradually changed gradually changed it's concept from a simple nutritional support to immune nutrition therapy.Glutamine can not only improve the body's protein levels of nutrition,protect the gastrointestinal mucosa,but also reduce the incidence of infection and the inflammation in the body,improve immune function,reduce side effects of chemotherapy,and at the same time enhance tumor sensitivity to chemotherapy and promote gastrointestinal cancer patients' postoperative recovery,reduce surgical complications.This paper aims to review the application and research of glutamine in the gastrointestinal cancer patients after the operation.

Objective To discuss the correlationship between the diagnosis of surgical pathology and the signs of malignancy which is both displayed by breast conventional mammography and amplificatory mammography with partial compression.Methods 65 cases with breast cancer were examined by both breast conventional mammography and amplificatory mammography with partial compression.The results of the two methods were analyzed.Results In all 65 cases of breast cancer,the comparison between breast conventional mammography and amplificatory mammography with partial compression could provide 16/60 cases in the X-ray signs of malignancy,which were in accordance with pathological examination results,19/41cases in the occurrence of calcification,13/37 cases in ductal calcification,0/4 cases in mixed type calcification,18/46 and 14/59 cases in the occurrence of lobulation and spicule signs.Conclusion Amplificatory mammography with partial compression can provide better insight in breast cancer than conventional mammography,especially in the region of dense shadow,small calcification and dense breast lesions,amplificatory mammography with partial compression can provide higher accuracy in diagnosis,especially for early breast cancer.

Objective To investigate the effect of traditional Chinese medicine retention-enema combined with tamsulsosin hydrochloride capsules in treating chronic abacterial prostatitis/chronic pelvic pain syndrome. Methods Two hundred patients of CABP/CPPS from Affiliated Hospital of Gansu University of TCM during September 2007-October 2012 were randomly divided into control group and observation group, 100 cases in each group. The control group received the treatment of tamsulsosin hydrochloride capsules, 0.2 mg/d, qn, and the observation group received the treatment of traditional Chinese medicine retention-enema additionally. The course was 4 weeks. The clinical effects of the two groups were observed and compared. The indexes including the scores of NIH-CPSI, leukocyte count and lecithin corpuscles in prostatic fluid, urinary flow rate were evaluated. Results The total effective rate was 100%(100/100) in observation group, and 88% (88/100) in control group. There was significant difference between the two groups (P0.05). Conclusion Traditional Chinese medicine retention-enema combined with tamsulsosin hydrochloride capsules can significantly improve the clinical effect of CABP/CPPS.

This paper explores the use of multi-channel event-related potentials (ERP) to identify mentality facticity or detect lie. Some identifiably meaning information, such as subjects' name and birthday, were selected as concealed information to be identified, 15 subjects were tested by concealed information test (CIT) paradigm and their electroencephalographs (EEG) were recorded from 30 electrodes. In virtue of analysis on the statistically significant difference between multi-channel ERPs evoked by probe information and that evoked by irrelevant information, the P300 amplitudes of 15 electrodes were selected as F-test samples. The significant difference of feature sample values between probe and irrelevant information was applied to identify mentality facticity. The results indicate that P300 amplitudes evoked in many electrodes are significantly different between probe and irrelevant information (P < 0.01). According to the significant difference of space sample values between probe and irrelevant information, the detection correctness to probe information reaches to 93.3% and is better than that of methods based single-channel ERP. The method proposed has the advantages of non-invasion and better accuracy, which could be used to identify mentality facticity effectively.

RESUMO

Gastrointestinal stromal tumors are the common mesenchymal tumors of the gastrointestinal tract. The constitutive activation of KIT is one of the earlies ttransforming events in GISTs and occurs mainly through activating mutations in the kit gene. PDGFRa is associated with the pathogenesis of GISTs too. The main mutative site of KIT gene are exon11, exon9, exon13 and exon17 and those of PDGFRA are exon18, exon12 and exon14.The clinical parameter and the response to imatinib of GISTs are associated with the site and type of gene mutation. Imatinib mesylate is a small-molecule inhibitor that selectively inhibit the tyrosine kinase and the clinical use of it is one milestone of the targeted therapy of GISTs. We review the gene mutation and targeted therapy development of gastrointestinal stromal tumors in this article.

Objective To investigate the clinical value of substitute popliteal vein valve operation (SPVSO) for primary lower limb deep vein insufficiency(DVIS). Methods The clinical data of 859 cases(907 legs) of DVIS who underwent SPVSO in the past 10 years in our hospital were analyzed retrospectively. Results Symptoms of edema and fatigue distinctly reduced or disappeared after the operation.Pigmentation in the lower portion of the leg began to disappear in 1 week, and nearly or totally disappeared within 2 weeks after the operation.Ulcer in the lower portion of the leg recovered three weeks after the operation. Patients were followed up for 6 to 120 months. The effective rate within 60 months was 96.8%, and the effective rate over 60 months was 87.2%. Conclusions The curative effect of SPVSO on primary deep vein insufficiency of lower extremity is definite and it is worthy to be clinical practice.

Objective To assess the clinical significance of the expression of p53, p21 protein in breast cancer (BC). Method The expression levels of p53, p21 proteins were examined by immunohistochemical (IHC) SP techniques in 69 BC specimen and 20 paracancinoma breast tissue. Result p53 and p21 proteins were failed to be observed in the paracancer tissue. p53 protein was found in 47.8% of BC, while p21 protein in 43.5% of BC. Along with the declined of BC cell differentiation degree, p53 expression increased significantly and p21 expression declined obviously. Objective To validate the molluscicidal effect of magnesium borate ore powder (MBOP), and explore the mechanism for providing the scientific basis for the field use. Methods The experiments were performed by the Copper Coin method, Volatilization method and Immersion and Sprinkle methods recommended by WHO. Results The MBOP can't kill the snail by the volatilization. The molluscicidal effect was well by the Immersion and Sprinkle methods. The acted effect was quick by using the Immersion method while the effect was more stable by using the Sprinkle method.LD_(50) of the Sprinkle method (27℃,4 days) was 4.07 g/m2. Conclusion MBOP's molluscicidal effectiveness is well, the action appears gradually and the effect is stable. The possible mechanism of MBOP might act through some dissolved components.

Objective To evaluate the safety of magnesium borate ore powder (MBOP) as a molluscicide, especially for the arsenic content in the soil samples from the fields used MBOP over more than one year. Method The arsenic in the soil samples was assayed by using the AgDDTC method recommended by the government. Result In all of the soil samples, the arsenic contents did not exceed the national health criterion. However, the arsenic in the MBOP treated soil samples was higher than that in the control one′s. Conclusion The application of MBOP as a molluscicide in the fields does cause some side effects on the environment.
